Dyslipidemia, characterized by abnormal levels of cholesterol and triglycerides, is a hallmark clinical feature in patients with Chronic Kidney Disease (CKD). This condition is not merely a secondary symptom but a fundamental risk factor that contributes to the accelerated decline of renal function and increased mortality rates associated with cardiovascular diseases.Lipid metabolism is directly impacted by the decline in renal function. In cases of renal insufficiency, there is a dysfunction in the enzyme "Lipoprotein Lipase" (LPL), leading to impaired clearance of triglycerides from the blood. In patients with Nephrotic Syndrome, the loss of albumin in the urine triggers the liver to overproduce lipoproteins to compensate for oncotic pressure, resulting in a sharp increase in total cholesterol and Low-Density Lipoprotein (LDL) levels. The primary danger lies in the fact that LDL cholesterol in kidney patients undergoes oxidation and chemical modifications that make it more atherogenic (prone to depositing in arterial walls). This leads to: Increasing the likelihood of myocardial infarction and stroke.Lipid deposition in renal cells (podocytes) triggers inflammation and fibrosis, accelerating the progression toward End-Stage Renal Disease (ESRD).Managing cholesterol in these patients requires an integrated approach starting with dietary modifications and reducing saturated fat intake. Pharmacologically, Statins are the first-line therapy. Studies have proven their efficacy in reducing cardiovascular risks; however, dosages must be carefully adjusted based on the Glomerular Filtration Rate (GFR) to avoid adverse effects such as myopathy.
